▪︎ What to Include in an Interview Evaluation Sheet

The specific items may vary depending on the role and hiring process, but a well-structured evaluation sheet generally covers the following:

  • Applicant information: name, position applied for, department, interview date and time
  • Interview details: interview round, interviewer name(s)
  • Evaluation criteria: relevant experience, job knowledge, communication skills, problem-solving ability, etc.
  • Rating scale: scores or grades assigned per criterion
  • Interview comments: specific responses, examples, or behaviors that support the ratings
  • Overall assessment: the candidate's strengths, areas of concern, and any points requiring follow-up
  • Hiring recommendation: recommend hire, consider further, or do not proceed

When deciding which criteria to include, focus on the competencies that are genuinely required for the role. Rather than adding as many items as possible, aim for a form that interviewers can realistically complete in full during or immediately after the interview.

▪︎ Two Common Mistakes When Completing an Interview Evaluation Sheet

1. Recording scores without documenting the reasoning behind them

Logging a score alone — for example, "Communication Skills: 4" — makes it difficult to reconstruct why that rating was given. Always pair scores with specific notes: what the candidate said, a concrete example they shared, or a behavior you observed. This makes it far easier for other interviewers or hiring managers to review the evaluation accurately after the fact.

2. Filling in the form well after the interview has ended

Right after an interview, you can recall a candidate's responses and behavior in detail. Wait too long, and those specifics start to fade. Where possible, establish a team norm of completing evaluation sheets immediately after each interview session. Documenting what was actually observed — rather than a vague overall impression — leads to more consistent and reliable assessments.

▪︎ A Simpler Way to Collect and Manage Evaluation Sheets

Is your team still asking each interviewer to fill out a separate spreadsheet or document and send it back by email or messenger?

With Shopl's [Report] feature, you can build a report form that matches your company's interview evaluation sheet, assign who completes it and when, and send a submission request directly to each interviewer. Submission reminders and progress status are all visible in one place, eliminating the back-and-forth of collecting individual files. Completed reports can also be downloaded and stored separately whenever needed.

If you want a more streamlined way to manage everything from evaluation sheet completion to submission tracking, give Shopl a try.
